- Self Contractor Questionnaire
- Lien Agent Form
- Affidavit of Worker’s Compensation Coverage
- Contractor License Check and Regulation Sheets:
- Plan Specifications (2 Copies)
- Erosion Control Certification
- Scope of Work
Manufactured Home:
(Single or Double Wide)
Please submit the following forms along with a copy of the Authorization for
Wastewater System Construction (septic permit) and well construction permit from the Appalachian District
Health Department:
- Mobile Home Permit Application
- Electrical Contractor License Check and Regulation Sheet
- Erosion Control Certification
Alteration Permit:
(Service change, heat change-out, well service, etc.)
Please submit the following application along with the correct sub-contractor form:

- Contractor License Check and Regulation Sheet
